gal has cardiomyopathy we won't know what kind the exact kind but judging by the fact she's pregnant and looks rather young and that ol grim is showing up it's probably something she had from birth and the reason grim is showing up is that the strain of pregnancy and birth is literally putting a fatal strain on her heart it's probably was a miracle enough already she got that far along in her pregnancy with the condition of her heart and grimy decides to at least give her the time needed to at properly bring her child into the world and at least see her child's face and give it a kiss like a mother should but apparently she was doomed to die anyways but at least she could die knowing her child face.

of course even more sad is while grim gives her the time to do that it does give some false hope to the father and the medical staff as maybe she'll pull thru and live but sadly as we see that isn't the case.
