This is totally unrealistic. Stories like this usually end with a double murder...but this is not even the problem here.

First problem: a drug strong to the point to make you confuse reality and dream, it's almost impossible to obtain and even so, it should have serious collateral effects on the victim.

But let's assume (unrealistically) that this drug exists and has no side effects on health, this brings to the second problem.

Second problem: "hypnosis" was added to condition her to react only from neighbour's dick and to not get off at all from mc's dick, no way the drug does that, drug can give you allucinations or twist your perception, but it surely cannot force in you beliefs you didn't have before. And yeah, while hypnosis actually exists in the real world, applications are very limited and things such as modifying the will of a person, are not possible.

But, again, let's assume (unrealistically) that they developed a way to easily and briefly brainwash a person. This brings to a third problem.

Third problem: Is it really that easy, to obtain all of these expensive drugs as a private person?

To overcome this point, let's assume they are backed up by a criminal organization like yakuza, mafia or whatever. They have the money and the materials. But they are only two people. Quite unrealistic. Usually organizations have to put some people inside to check their subordinates' work, and help them in such work. Never hear about clans with only 2 people.

But hey, it's hentai, so let's say they work alone anyway, and their higherups give them all the money and stuff without questioning.

Assuming all this unrealistic stuff, both MC and wife must be the dumbest amongst the dumb (what they "suffered" is not totally irreversible, he still has the dick and the vessels to bring blood to the erectile parts, so with surgery you can have erections again probably, and she can, with disintoxication, snap out of her condition), in top of that, to not even try to get out from this situation, but even accepting it.
