If I'm not mistaken, after getting vanilla ending with his waifu, he was going to put the medals away but keep it nearby as reminder of what he had done.

Then the medal sparkle and turned to dust, as if to say "nah, don't worry about it and get on with your life"

And he thanked it for...well...the whole experience in this story I guess (the power trip the gender bend? the moral at the end)

The D-Medal appears to have four modes: DESIRE is the initial form; it gives its holder the ability to charm girls into having sex with the holder. When the medal is used frequent enough, DESPAIR mode activates; it transfers the holder's consciousness into a female body that's vulnerable to D-Medals while "something else" assumes the identity of the holder. Although the holder remains a girl, he can be given a chance to return to his normal self whenever he feels the need to protect a girl, which activates DEFENSE mode. I'm not sure about this last part, but having sex with a girl that he truly likes seems to contribute to the last mode of the medal, DISABLE, which finally destroys the coin, having fulfilled its purpose.
