This looks a lot like Kyou no Yuiko-san, a manga that I always believed that was drawn by someone skilled in the EROS department.

Is there supposed to be a joke with the note that didn't translate? It looks like the text was originally laid out so that it could be read either vertically or horizontally. That would explain the mix-up - he wrote the note vertically, but she read it horizontally by mistake and got a completely different meaning.
