Okay, not quite sure what happened in Chapter 5. Our MC is starting to lose hope in returning to normal, but as soon as he sees his crush (still immune to the D-Medal) at the brink of being raped, he was able to appear as himself again. It seems that the medal persona sees his sincere efforts to save his crush and decides to give him one more chance, but will be watching out for when he'll use the D-Medal again.

Hopefully, the translations would clear and/or confirm some things.

I don't really read all the text because I'm a slow reader but going by the textboxes in p133, looks more like MC gets his body back and the 'medal persona' hints that he's going to do this again to someone else, and that he's done with the MC probably because he's reformed.

Relevant lines:

「もう逃げんじゃねーぞ」
「さて…もう少し楽しみたかったが」
「元のあきかね君に戻るか」
「次はどんなやつがメダルを使うのかねえ」

My shitty translation:

"Stop running away (from whatever it is)" (probably at his feelings for kansaiben girl)
"Now then, I wanted to enjoy this for a little longer but..."
"It's time to turn back into the original Akikane" (I'm guessing akikane is MC's name)
"I wonder what kind of guy the next user of the medal's going to be?"

edit: I also have no idea why kansai is immune to medal but I don't really care either way. At first (without reading) I thought fake MC was going to NTR MC by saving kansai and fucking her but now probably happy end.
