Regardless of whether or not this is NTR, I think it's pretty obvious why people are tagging it as such and not just doing so "to be a bastard": it has NTR in the title. There are exceptions, but (especially in this genre) authors tend to title their works for marketing purposes, so if you see something called "Big Breasted Married Woman Gets Fucked by Five Old Men", you're pretty sure what you're getting into. In an ambiguous tagging case like this, most people would end up siding with the author.

Also, I'm not going to debate what the NTR mojo is, or whether e-hentai's tag for it is properly defined, but the tag definition says "Another person's spouse / partner being unfaithful and the one being cuckolded being aware of the situation", which is clearly the case in this doujin.

No, what you are describing is cuckold, the feeling of arousal and most of the time the male and female should be consent.
