The virtual world system could set aside a flag for that purpose but keep all these secret flag statuses secret from the users. Thus, the gov't could use the virtual world system as a sort of simulator to see how diseases propagate without harming anyone. And with the status being secret, the users won't know to complain about being test subjects.
