@Cdub7888 Nah, it's just that she's the resident genius pharmacist, and the overall neutral position of the Eientei faction makes her easy to shape as a villain. That, and she's lunarian, so she might not have the same morality scale. Your mileage may vary, just like for any Touhou char.

She's also done some pretty questionable things in the past, like murder the Lunar emissaries who were sent to let Kaguya know that she was forgiven for her sin, or even the creation of the Hourai elixer in the first place, a great sin in and of itself. Plus she bullies Reisen in Inaba of the Moon & Inaba of the Earth, though that's questionably canonical and played mainly for laughs, so whatever. Point being, she's a ruthless lady who's willing to employ a frightening mind to achieve her goals.

Eirin is for the most part a pretty nice lady in canon. She was indeed somewhat ruthless back in the days but based on what I've read (Cage in Lunatic Runagate and other official materials I've seen), even after all the years she lived, she matured over the events of the Touhou canon. She had some compassion for a man who came on the Moon and while he did die later after coming back on Earth, he was able to die in peace thanks to her. Heck, the Watatsuki sisters that are usually hated by the fandom were the ones that allowed this man to stay.

The main thing with Eirin is that it's fair to say Eirin is a "magnificent bastard" who is often ahead of everyone in term of intellect (the only way she's outdone is by Yukari at the end of Cage in Lunatic Runagate). Her behaviour towards Reisen in Inaba of Moon & Earth is one that is fair to consider exaggerated. It's fair to assume she punishes her but that book has a strong emphasis on humour. While she did do some questionable actions in the past, today in canon she's a helpful doctor who helps patients and charges them little and even lets them pay later (you better pay at some point though).

The reason why she's often portrayed the way we see in doujinshi like these in my opinion is that first of all, she's the perfect character to use as an excuse to have such drugs. As for her more sadistic behaviour, it is most likely an even more exaggerated behaviour based on how she is with Reisen. The thing though, in canon, she strongly care for her (Udonge is a more heartwarming nickname than one might think once they read Cage in Lunatic Runagate) but she's a very demanding and difficult master/teacher.
